Ursula's Costumes Inc. has made a strong commitment to giving back to our local community.

Due to the number of requests we receive, Ursula's Costumes Inc. is only able to assist organizations once a calendar year. Please be aware that we only fulfill two requests and cannot guarantee that every donation request will be met.

Donation Guidelines

• Guidelines for submitting a donation request to Ursula's Costumes Inc. are as follows:
• All requests must be typed on your organization's official letterhead
• Requests must be received no later than six (6) weeks prior to your event date
• Requests must include the following information:
  • Organization's name, 501(c)(3) tax id number, and mailing address
  • Event name, date, and location
  • Type of fundraiser
  • Who/what is benefiting from the event/funds
  • Contact name, mailing address, and phone number
• Mail requests to the following:

• Donation requests will be responded to via mail only, one to two weeks prior to your event. Faxed, phone, e-mailed or incomplete requests will not be accepted.
